Abstract

Dementia is often viewed through the lens of progressive decline, with care becoming increasingly focused on managing loss rather than supporting people to continue living meaningful, purpose-filled lives. Yet a diagnosis does not mark the end of identity, purpose or connection. What if we approached dementia differently, not as a journey defined by decline, but as one that could still be lived with dignity, meaning and joy from diagnosis through end of life? Drawing on evidence-informed international practice, specialized training, and real-world experience in Barbados, this presentation introduces a Caribbean approach that bridges internationally recognized dementia practices with the cultural realities of Caribbean families and communities. Integrating Therapeutic Recreation, DementiAbility Methods™, U-First®, Palliative Care for Dementia and Death Doula principles, it demonstrates how meaningful engagement evolves alongside the person rather than disappearing as dementia progresses. At the heart of this approach is the belief that every person living with dementia continues to have the capacity to experience purpose, contribution, connection and joy. While grounded in Caribbean values of family, spirituality and community, the principles presented are adaptable across diverse cultural and care settings. Through practical case examples, including familiar routines, faith-based activities, music, baking, community outings and legacy conversations participants will discover how everyday occupations become powerful therapeutic tools for preserving identity, strengthening relationships, reducing distress and enhancing quality of life. The presentation also explores the value of introducing palliative principles and legacy work early in the dementia journey, enabling individuals to actively participate in preserving their stories, values and wishes while preparing and supporting families through each stage of care. Attendees will leave with practical, person-centred strategies that can be implemented within homes, community programmes and residential care environments. More importantly, they will be challenged to rethink traditional models of dementia care and embrace an approach where meaningful engagement does not end as dementia progresses; it evolves, affirming that although dementia changes the journey, it should never diminish the persons humanity.
